Review Packs and Operating Reports

Two review records, deliberately separated: an issued operating paper for the meeting and a working commentary log that prepares explanations and carries open actions forward.

Both examples use purpose-made operating records. Their displayed counts agree with the saved workbook, presentation and decision log.

One record, five review stages

From material movement to carried action.

The working log preserves the question and response. The issued review preserves the decision. Together they show the complete management route.

Two review settings

The paper and the log have different control roles.

The formal pack is selective and issued. The commentary workbook is operational and cumulative. Treating them as one document would hide the evidence trail and weaken the meeting record.

Commentary is requested by a rule, reviewed by a person and retained with the decision.

Review discipline

Selective reporting still needs a complete trace.

The format changes with the meeting. The discipline does not: a reader should be able to see why an item appears, what was asked, what was decided and who owns the next move.

    Materiality sets the agenda.

    Movement, risk or uncertainty enters the review when it could change a management decision. Supporting detail remains available without competing with the main question.

  2. 02

    Narrative explains the movement.

    A useful comment states what changed, why it matters and what should happen next. It does not repeat the number shown beside it.

  3. 03

    The decision has an owner.

    Recommendations, approvals and follow-up actions name the responsible role and the point at which the matter returns for review.

  4. 04

    Limits stay beside the decision.

    A known timing, data or workflow limitation remains visible beside the decision it affects, not hidden in an appendix.
